One yearly fee for routine visits, with the terms printed beside it. It is not insurance, and it can't be used with insurance. The prices are examples the practice replaces with its own.

The terms
What the terms mean
Printed on the card, and here in plain words.
Term and renewal
You pay once, for twelve months. The plan renews each year unless you cancel before the renewal date, and we remind you first.
Cancelling
Call or write before the plan renews. There's nothing else to do.
Refunds
Cancel within 30 days of joining and we refund the fee, less the value of any visits you've already used.
Not insurance
The club is a yearly fee for visits at this office. It doesn't pay claims, it isn't a Medicare plan, and it can't be combined with insurance.

Visits beyond the two a year
An extra visit is quoted before it starts.
Insurance or Medicare coverage
The club doesn't replace either. If your plan has a dental benefit, ask it what it pays first.

Can my spouse and I join together?
Yes. The two-member plan covers two adults in one household.
Is it a good deal for me?
It depends on how often you come. Ask Noe for the club's visit list and the fee for a single visit, and compare them at home.
What happens at renewal?
We remind you before the renewal date. If you do nothing, the plan renews for another twelve months.
